To make a driver verification request, please select one of the following options:

Internet

1. Go to www.driverfacts.com
2. Login or create a User ID.
3. Search for SSN.
4. Select and purchase your report type.
5. Print and fax the barcode and D&A release to the number on the form.
6. Your results will be available the same day via the internet.

Non-Internet Request Form by Fax or Mail

1. Call toll free (888) 844-4730, press option 6.
2. Enter your fax number to receive a DriverFacts Non-Internet Request (NIR) form.
3. Fill out the form. Enter SSN, report type, and payment information.
4. Fax the NIR form and Drug & Alcohol release to the number on the form.
5. Your results will be faxed to you in 1-2 days.

Mail (Drug & Alcohol and Accidents No Charge)

1. Go to www.driverfacts.com
2. Login or create a User ID.
3. Search for SSN. Select company the applicant worked for.
4. Click Order Report. Select Drug & Alcohol Report by U.S. Mail.
5. Fill out the online form.
6. Print the form and follow instructions.
7. Mail it to the specified address on the form.
8. Allow several days for the results. They will be mailed to you.
